Welcome to the Agua Fria Union High School District! This is a brief summary of the benefits provided to full-time employees (.6667 Certified or 30 hours per week) for the plan year beginning July 1, 2012 through June 30, 2013.

Standard Life Insurance - District Paid In the event of your death, your beneficiary will receive a death benefit equal to one (1) times your annual salary.
Standard Life – Additional Term Life Insurance Standard Life is portable Term Life Insurance for the employee and family.
Texas Life Texas Life Insurance is portable permanent individual life insurance for the employee and family.
ASBAIT Medical-PPO This plan provides access to the Blue Cross/Blue Shield Provider Directory. You may see any physician in the directory without a referral. By receiving your care and services from a provider in the BCBS network, you will receive a higher level of benefit and therefore have less out-of-pocket expense. If the provider is in-network, you will only be responsible for the co-pay, deductible or coinsurance as noted in the ASBAIT schedule of benefits. Employee medical benefits begin on the first day of full-time employment.
ASBAIT Dental - PPO
Ö Employees may choose any dentist. Ö Routine Cleaning – Twice in a calendar year, covered 100% Ö Basic Services covered at 80% after a $50 deductible Ö Major Services covered at 50% after a $50 deductible Ö Maximum benefits per person, per Calendar Year is $1,500 (all dental and orthodontic services combined) Ö Orthodontic benefits covered at 50% with a maximum lifetime benefit of $1,500 (Coverage is provided only up to age 19).
EMPLOYERS DENTAL SERVICES (EDS) - DMO Ö Managed Care Plan. You must choose a participating dentist who will be responsible for all your dental needs. Ö Routine services will be provided at reduced fees called co-payments. Ö EDS Orthodontists offer 25% off their normal and customary fees.
AVESIS Vision Plan
Ö Employees may choose from a network of vision care providers. Ö Complete vision exam – once every 12 months - $10 Co-payment Ö Lenses – once every 12 months Ö Contact Lens Allowance – once every 12 months – covered up to $130 Ö Frames – once every 24 months - $25 Co-payment. Any frame with an approximate retail value, between $100 - $150 ($50 wholesale) Ö Up to 20% off on all additional purchases or items not covered
American Fidelity Assurance Company
Ö A $200 to $7,500 per month benefit is payable, depending on the benefit level you select, when you are totally disabled (maternity is covered same as an illness) as a result of covered sickness. Ö Benefits begin on the eighth (8th) day of total disability and continue while you are disabled, but not to exceed 180 days for any one total and continuous period of a disability. Benefits are paid year round, even when school is not in session. Ö Monthly benefit pays up to 66 2/3% of your salary.
Arizona State Retirement System (ASRS) Long-term Disability (LTD)
Ö 2012-13 contribution is .24% of wages Ö Provided through your participation in the ASRS. Ö Plan pays 66 2/3% of your monthly salary after you have been disabled for 180 days. Ö Your LTD benefit will continue until your recover, are no longer under medical care, retire, or are no longer eligible for benefits under ASRS guidelines, whichever comes first.
First Financial Group of America Flexible Spending Accounts (FSA) allow an employee the opportunity to put some of his/her salary aside before taxes to pay for many common out-of-pocket expenses.
If you do not spend all the money you have elected, it will be forfeited because it cannot be rolled over or refunded to you. Any money that you elect to set aside in a flexible spending account for a given Benefit Period may be used only for eligible expenses you incur for services received during that Benefit Period. So, it’s very important that you plan carefully when deciding on how much to allot for your FSA account(s). There are two types of FSA plans:
MEDICAL REIMBURSEMENT ACCOUNT: Money from this account can be used to pay for certain health care expenses that are not reimbursed by any other source. Examples of this include plan deductibles and co-payments, items the group plan may not cover such as well baby care and vision expenses, as well as uncovered dental expenses, including orthodontia.
DEPENDENT CARE REIMBURSEMENT ACCOUNT: Money from this account can be used to pay for certain dependent care expenses which are incurred so that you (and your spouse, if married) are able to work. The availability of funds for your Dependent Care Reimbursement Account will be based on monies accrued through payroll deductions. Qualified dependents are children under the age of 13 and dependents of any age who are physically or mentally unable to care for themselves.
PREMIUM ONLY PLAN: The Premium Only Plan allows you to pay with pre-tax dollars your portion of the premium(s) for the benefits offered as part of the Flexible Benefit Plan. The program requires participants to make an election prior to the start of each plan year or when newly hired. This deduction is accommodated through payroll deduction. Your taxable income is reduced by the amount of premiums paid on a pre-tax basis resulting in lower taxable income. These premiums are not subject to FICA tax, federal tax, and state tax.
Arizona State Retirement System (ASRS) The Arizona State Retirement System contribution for 2012-13 is 10.90% of wages. This 401A Plan is designed to provide supplemental income to you upon retirement. The amount of contribution is shared equally by you and the Agua Fria Union High School District. Cancer Insurance Cancer Insurance is designed to be a supplement and pays for many of the costs not covered by your major medical insurance. This plan pays in addition to other coverage you may have. This plan also helps pay for a portion of your yearly wellness exam.
Accident Insurance – American Fidelity You cannot predict when or if an accident will happen, but you can protect yourself and your family against the high cost of an accidental injury or death. This plan pays in addition to other coverage you may have. Family coverage is available.
Long Term Care – American Fidelity The need for Long Term Care can happen to anyone. Anyone can be affected by an illness or accident, not just the elderly. Long-Term Care Insurance is a good choice at any age.
Worker’s Compensation Employees are automatically covered by Worker’s Compensation insurance through the district’s current worker’s compensation carrier. If an employee is injured in an accident arising out of and in the course of his/her employment, he/she is entitled to 66 2/3% compensation.
Tax Deferred 403(b)/403(b)(7) Investment Plans Internal Revenue Service laws allow employees of public schools to participate in tax-sheltered 403(b)/403(b)(7) plans to set aside savings for retirement on pre-tax basis. Employees may initiate participation at any time. The Agua Fria Union High School District has appointed First Financial Administrators to be the Third Party Administrators for our 403(b)/403(b)(7) plans.
Leaves Full-time employees (those working 30 hours or more) qualify for a variety of leaves, including discretionary, sick, jury duty, military, family and medical, parental, bereavement, and absences for other causes approved by the Superintendent.
