The Mollen Clinic will be coming to each site to administer the H1N1 Vaccine to registered students and staff that have pre-registered or bring in the signed consent.

How do you register, you ask?? 
Follow these three simple steps:
1.  Go to www.FluShotsUSA.com/schools 
2.  Select your School district and school.  Enter information as prompted.
3.  Print out the confirmation / consent form.
-District Office and Transportation staff may participate at any site.
-The Mollen Clinic accepts most insurance, check their website for information. 
-Employees insured with ASBAIT will have $0 co-payment.

Please let your students know that their parents can now find these instructions on our district website as well as each school website.
