ATHLETICS PACKETS

All students participating in athletics must complete the Athletic packet.  When returning the completed Athletic packet, the following items must also be turned in:

• proof of insurance or purchase of school insurance
• athletic participation fee
• physical exam form (please use the AIA physical form included in Athletic packet)
• Concussion Awareness Training confirmation certificate
• copy of birth certificate or equivalent proof of birth date and place of birth

Students will be unable to participate in tryouts, practices, or games until cleared by the Athletic office.
