    This is a brief summary of benefits provided to full-time employees (.6 FTE Certified or 30 hours per week for classified) for the plan year beginning

     

    July 1, 2016 through June 30, 2017

    UNITED HEALTHCARE MEDICAL PLAN

     

    The District currently offers eligible employees three medical plan options. The plans include Choice Plus ONE; Choice Plus TWO and a Choice Plus HDHP. Understanding the differences between the medical plan options can help you decide which plan is best for you and your dependents.  These plan options are preferred provider organization (PPO) plans accessing the United Healthcare network providers.  AFUHSD pays $394.00 per month towards one of the three medical plans you elect.  If the employee waives medical coverage, the District will pay for employee ONLY coverage for dental and vision insurance.  With PPO plans, you can see any provider you wish, but you get greater cost savings by using the in-network PPO physicians, hospitals, labs, and urgent care centers.  This is important to consider since you share in the cost of care in the form of copays and coinsurance.  For more detailed information on plan structure, Click Here:

    DELTA DENTAL OF ARIZONA

     

    The Delta Dental plan covers preventive services at 100% and this service does not apply toward the annual maximum benefit. The services include oral cleanings, examinations, and X-rays.  Once you start using additional dental services, your annual deductible applies ($50 single coverage/$150 family coverage).  Basic Services are covered at 80% and Major Services are covered at 50%.  Orthodontic Services are available for children, 8 to 18 years old.  To find a dentist Click Here:  For more detailed information on plan structure, Click Here:

    EDS DENTAL DHMO PLAN

     

    EDS is a managed care plan where employees either choose a dentist or are assigned a dentist within the provider network: www.mydentalplan.net. Routine services will be provided at reduced fees.     AVESIS VISON PLAN

     

    Comprehensive vision plan that allows a vision exam once every 12 months, at very low cost.  Lenses and contact lenses are once every 12 months, frames once every 24 months.     LIFE INSURANCE

     

    Symetra Life insurance is the carrier for the District paid life insurance benefit. Full-time employees are eligible for a death benefit equal to one (1) times their annual salary upon. Additional life insurance as a supplemental life insurance for Spouse (25K max), Children (10k max) and Employee (150k max) is available for election during Open Enrollment.

     

    Other Benefits

     

    Arizona State Retirement System (ASRS): The Arizona State Retirement System contribution for 2016-17 is 11.34% of wages. This 401A Plan is designed to provide supplemental income to you upon retirement. The amount of contribution is shared equally by you and the Agua Fria Union High School District.     Arizona State Retirement System Long Term Disability (LTD): The ASRS Long Term Disability contribution is .14% of wages which is provided through your participation with ASRS. The plan pays 66 2/3% of your monthly salary after you have been disabled for 180 days. Your LTD benefit will continue until you recover, are no longer under medical care, retire or are no longer eligible for benefits under ASRS guidelines, whichever comes first.

     

    Worker’s Compensation: Employees are automatically covered by Worker’s Compensation insurance through AZ Alliance for Worker’s Compensation. If an employee is injured in an accident arising out of and in the course of his/her employment, he/she is entitled to 66 2/3% of their regular compensation.

    Tax Deferred 403(b)/457(b) Investment Plans:  Internal Revenue Service laws allow employees of public schools to participate in tax-sheltered 403(b) or 457(b) plans to set aside savings for retirement on a pre-tax basis. The district also offers a post-tax investment option with a ROTH 403(b).  The Agua Fria Union High School District has appointed TSA Consulting Group, Inc.  to the be the Third Party Administrator for these plans.     Leaves: Full-time employees qualify for a variety of leaves including discretionary, sick, jury duty, military, family and medical, parental, bereavement and other absences as approved by the Superintendent.
