United Healthcare PPO MEDICAL AND DENTAL PLAN
The United Healthcare PPO Plan provides pre-tax Medical and Dental coverage. The district pays $394.00 per month towards one of our medical plans (Choice Plus One, Choice Plus Two and HDHP1300). If the employee waives medical coverage the District will pay for employee only coverage on both dental and vision insurance. Premium sheet, Summary of Benefits, Enrollment Guide, Health Savings Account (HSA) forms are attached and found under documents.

EDS DENTAL DHMO PLAN
Managed Care Plan where employees either choose a dentist or are assigned a dentist within the provider network: www.mydentalplan.net. Routine services will be provided at reduced fees. EDS Orthodontists offer 25% off their normal and customary fees.

AVESIS VISION PLAN
Comprehensive vision plan that allows a vision exam once every 12 months, lenses once every 12 months, contact lenses once every 12 months and frames once every 24 months. There is up to 20% off of additional purchases that are not covered under the plan.

Symetra Life insurance carries the district paid policy. Full-time employees are eligible for a death benefit equal to one (1) times their annual salary upon election during Open Enrollment only. Additional life insurance as a supplemental life insurance for Spouse(25K max), Children (10k max) and Employee (150k max) .
Arizona State Retirement System Long Term Disability (LTD): The ASRS Long Term Disability contribution is .24% of wages which is provided through your participation with ASRS. The plan pays 66 2/3% of your monthly salary after you have been disabled for 180 days. Your LTD benefit will continue until you recover, are no longer under medical care, retire or are no longer eligible for benefits under ASRS guidelines, whichever comes first.
